Service Coordination

 

In Newberry County, individuals suspected of having or diagnosed with mental retardation or autism enter the service delivery system through service coordination.  Service Coordinators serve families as the single point of entry as well as act as a continuing contact person, resource, and advocate.  The Service Coordinator strives to assist those served to access services both within the community and the DDSN service delivery system.  We recognize that our service recipients have the same needs as others, but sometimes may need assistance with locating, accessing, and coordinating services. A plan is developed for each consumer which focuses on needed and desired services for the upcoming year.   The Service Coordinator identifies resources to meet individualized needs and assists with arranging services as identified on the individual’s plan.  The Newberry County Disabilities and Special Needs Board employs Service Coordinators to help establish, arrange for, and monitor an array of services needed and received by the service recipient.  Such services may include medical, vocational, educational, recreational, residential as well as other services.
