 Direct Support Professional Position Description:

Duties:

Under the general supervision of the Direct Support Supervisor or Residential Manager, supports people to achieve their highest level of independence.

  

Essential Functions

 

  1. Supports people based on information gained through conversations with the person and their plan of support. Develops a working relationship with people that provides individualized training in all skills based on the individual’s personal goals.

 

  1. Communicates effectively with other departments to ensure appropriate supports are addressed.

 

  1. Supports people to experience a variety of community options and settings.

 

  1. In coordination with the person and the plan manager, helps to develop goals and objectives based on the individual’s choice and assessments.

 

  1. Provides supports to people in the area of personal care and daily living skills.

 

  1. Assists persons being supported in learning appropriate behaviors through positive behavior support.

 

  1. Performs and supports people in the preparation of meals through individual choice and/or as outlined by agency or state regulations.

 

  1. Ensures the health safety, and accountability of people being served as indicated in agency policies. Conducts safety drills and inspections as required. Regularly discusses safety issues with people.  Ensures that people have the best possible health.

 

  1. Supports people by attending life plan meetings when invited.

 

  1. Maintains transit safety through completion of transportation logs, daily vehicle checks, and adherence to agency safety policies.

 

  1. Maintains appropriate documentation as required.

 

  1. Provides a clean, safe, and respectful environment for people though support and training.

 

  1. Performs other related duties as required.

Duties: Serves children ages birth to 6 and their families. Develops, coordinates, reviews and revises the individual family service plans. plans and implements services; other duties as required.

Requirements: Minimum Qualifications include Bachelor’s Degree in Early Childhood Education, Child Development and Family Studies, Special Education, Social Work, Sociology, Psychology or related field from an accredited college or university and one year experience working with children between birth and age 5

Acquire and maintain SC early interventionist credential, possess a valid SC driver’s license with good driving record. Good oral and written communications skills.

COMMUNITY TRAINING HOME / ICF  LPN NURSE

FULL TIME

                                                                                

DUTIES :   UNDER THE SUPERVISION OF THE LEAD LPN, ENSURES THAT COMMUNITY TRAINING HOME INDIVIDUALS, AND INTERMEDIATE CARE FACILITIES (ICF) INDIVIDUALS, HAVE PROPER NURSING CARE.  ASSISTS IN PROVIDING OTHER NURSING NEEDS TO THE AGENCY.

          

 

                                            

REQUIREMENTS:   CURRENT LPN LICENSURE WITH S.C. STATE BOARD OF NURSING. EXPERIENCE WORKING WITH SPECIAL NEEDS POPULATIONS PREFERRED. ABILITY TO WORK INDEPENDENTLY.  GOOD DRIVING RECORD
